Purpose

The goal is safe, steady weight loss. Increasing the dose slowly reduces side effects, protects muscle mass, and prevents the kind of rapid weight loss that leads to fatigue, weakness, and nutritional deficits.

Injection Frequency

Inject once every 5 days and keep the spacing consistent.

Injecting every 5 days instead of once a week keeps the medication steadier in your system. This means more consistent appetite control, fewer side effects, and less of that "wearing off" feeling before your next shot.

Starting Dose

Begin at 10 units (1 mg) for your first two injections.

Titration Structure

After the first 10 days, increase the dose every third injection (every 15 days). Each increase adds 10 units (1 mg).

Continue this pattern until you reach the dose that delivers steady results.

Maximum Dose — Titration

90 units (9.0 mg) per injection. Never exceed this amount. Most people titrating from scratch will not need more than 50 units (5.0 mg).

If you are currently on tirzepatide, multiply your current weekly dose by 0.6 to find your retatrutide dose every 5 days.

Weekly Weight-Loss Rule

Weigh yourself every 7 days and use this to guide dose changes. In the first two weeks, weight loss of 3 to 5 lbs is common due to water and glycogen changes — this is not a reason to delay your first escalation.

Increase dose at the next scheduled interval.

1 – 2 lbs

Ideal range. Continue or hold at this dose.

> 2 lbs

Hold 15+ days. If rapid loss continues, reduce by 10 units.

Protecting Muscle Mass

Weight loss medications reduce appetite, which can lead to inadequate protein intake and muscle loss. Two things protect against this:

  1. Protein — At least 1 gram per pound of your target body weight, every day. Prioritize protein at every meal.
  2. Resistance Training — Lift weights or do bodyweight resistance exercises at least 2 to 3 times per week.

Non-Negotiable

Without adequate protein and resistance training, a significant portion of weight lost will be muscle rather than fat.

When to Stop Escalation

  • You reach your target weight
  • Weekly loss is consistently 2 lbs or more
  • Persistent nausea or stomach discomfort
  • Fatigue, weakness, dizziness, or dehydration
  • Difficulty eating normal meals
  • You reach the maximum dose (90 units / 9.0 mg)

Maintenance & Missed Dose

Stay at the dose that controls hunger, holds weight loss at 1 to 2 lbs per week, and avoids side effects. If weight loss becomes too fast or symptoms develop, reduce by 10 units and reassess after 15 days.

If you miss a dose, take it as soon as possible and resume the 5-day spacing from that point. Do not double a dose.
